Payment
Mid Atlantic Retina requests payment in full at the time of service. Patient responsibility includes co-payments, deductibles, and coinsurance. Cash, check, American Express, Visa, Mastercard and Apple Pay payments are accepted.
We will submit all insurance claims on your behalf, in which we are a participating provider. Required information includes:
- A copy of your insurance card
- Photo ID
- An address and phone number to submit claims and verify coverage
We ask you to please review your insurance for deductible or coinsurance amounts that may apply at the time services are rendered and payment will be collected at the time of the office visit. Mid Atlantic Retina will not become involved in the disputes between you and your insurance company regarding deductibles, co-insurance, covered charges, additional coverage’s, etc. We will supply requested information as necessary for further processing.

Insurance
We will need your insurance information prior to scheduling your first appointment so that we may verify your benefits for services rendered.
- Mid Atlantic Retina will submit claims to all insurance carriers, in which we are a participating provider. We request that patients bring their health insurance cards with them to appointments.
- If a referral is required by your insurance carrier, it must be provided at the time of service. Referrals must be obtained from your Primary Care Physician.
- Any balance, after payments at the time of service and from processing of insurance claims, will be billed to the patient and must be paid within 20 days.
